I have no idea what a dev that needs permission to release his own game is supposed to mean.
While Renpy (the most common VN engine seen here) can do great things in 2D with overlays and such, if you want 3DCG it means a fresh render of every possible version of a scene. Giving you two different choices of MC avatar means doubling the number of renders for any scene with the MC in it, and most VNs are done from a 3rd person perspective. Short answer: it takes too much effort.
