You CAN get them all pregnant. All four of the main girls during the game (sisters & mom), and the rest in the aftermath. Bella and Liza have dialogue options where they'll talk to you about it. Rachel has an option where she'll tell you she forgot to take the pill and is pregnant. The Mom will just turn up pregnant after the flash forward.

I enjoyed the game the only things I didn't like were James and the lack of any animation the harem route is hilarious it's like Oprah popped out of nowhere and started shouting "YOU GET A BABY AND YOU GET A BABY ALL OF YOU ARE GETTING BABIES!"
