Was stuck on that one for a long time, but eventually figured it out sort of on accident. One of the ladies that runs the gym (the one with the red ponytail, not the frizzy red hair) has some history with the gym guy. If you build up some friendship points with her, she will tell you some about it and agree to join his gym, and that should give you 4/4 and progress it.

Once you reach a high level (courage) things become too easy for Miss Nash, I personally chose to "punish" her with a sense of guilt and shame that she overcomes by drinking liquor after every time she sells her body. It would be nice if, having achieved intimacy with the other characters, there was the option "Greetings with a kiss" not "I'll come later", perhaps it is too much to ask to level it: a hug, a kiss, a passionate kiss.
