I like the game, but maybe the trans content should be its own separate game, maybe Midlife Identity Crisis. It's just that the trans content does not really fit with the main game story IMO, there is not really any integration of the trans characters into the family dynamic like some of the other characters.
Just food for thought.
Integration would have made it impossible to make optional. There are more people put off by trans content than turned on. Dev already put the character on rails - you can turn her away a dozen times and the next scene you're right back there trying to have sex with her anyway.

what can I do
Gay tags are often confusing. It's mean lesbians, trans , futa or just mens action. So how is with this game?
There's a trans character, and while no matter what you do you can't avoid them (seriously, take all the worst choices and in the next scene you're still on top of them!) but you can remove the penis and avoid some 'receiving' scenes. There's also lesbian and loli stuff. Technically NTR (I think, not 100% clear on the definition), but it's the MC cucking someone else (who gets off on being cucked).
You don't have to interact with trans characters in game. Sorry. You did something wrong.
There is also a MMF threesome with sis and her BF. You can choose that or a FFM threesome with nasty looking wife and sis or you can opt out of the scene and get nothing with sis.
