The scenes are the same with or without the patch. It only changes your relation to the girls making you related.
The incest patch just change the relationship of the characters. I.e. instead of Landlady it will be Mom or Mother, or Landlord will be Dad or father, or Roommate will be the relationship between siblings, either brother and sisters or brother and brother
