There is no incest in the game without incest patch. And that patch is made by a regular player, not by the dev.
Mac fix:
After you unzip the file, right click the game and select "show package contents", inside the Content folder open the MacOS folder. Right click the file that says "MilfyCity" and copy it. Open the app Terminal. Type: chmod +x and then paste without a space after the x, hit Enter. Then click the original icon for the game. It will bounce for a while before giving up. Dismiss any message that pops up and open system preferences then security and privacy. Allow the app to "open anyway" and that should do it.
