No NTR at all. Just make sure to break up your older sister and her boyfriend, since he's cheating on her. (Which you're highly likely to do anyways.) I recommend following the walk through. The game isn't difficult without it, but you can ensure you get the BEST setup for the next version.

how to get all the girls pregnant at the same time?
Sleep with all the girls, and come inside all of them. Follow the walkthrough for more information, but the general gist of it is that you need to talk with them about it being okay for them to be pregnant when that conversation comes up. (Bella's/Liza's are fairly obvious, but the Mom's is less so.)
If you blackmailed and dominated Bella she won't get pregnant, is your relationship status is 'in love' she'll conceive. Same goes for Landlady/Mom, if your relationship is you being submissive, she won't get pregnant, if you have 'in love' status she'll conceive. Other two also need 'in love' relationship status. That's it.

To me it's definitely the most complete feeling incest game on here, and that's saying a lot: there's a bunch of them. (and, this one isn't even done too!)
