At the start there is a choice, and it is not entirely clear. You either tell the doctors about hearing voices (cuck route) or you do not (bull route).
Note as the bull you do in fact cuck someone else. Also there is one optional cuck scene where you can just choose to intervene and avoid it even in the bull route.
