The gay filter only disables most of the fullscreen gayporn images, and disables most gay dialogue options for the player.
But the other gay scenes still happen. Like your mandatory buddy Eric always gets fucked by other guys. Elves are always futa and the MC gets Rough Sexd when losing combat.
And most women look like ridiculous bimbo surgery accidents. So you kinda wish their scenes were hidden, too.
